Payment of your office visit is expected at the time of your visit. If you have questions regarding our fees, feel free to discuss them with our Patient Account Coordinator or our Administrator.

We bill your insurance company for surgical procedures and some office calls depending on what type of insurance you have.

If you are not covered by insurance and have financial difficulties, please call and discuss payment arrangements prior to your visit. After verifying your financial situation, we may be able to arrange a payment plan. We will be happy to assist you with your account, but please do not ignore your financial obligation. Give us a call or stop by the office to work out a plan suitable to both the office and you. Failure to do so may result in collection procedures.

This office is participating with Medicare, Medicaid, Blue Cross/Blue Shield "Traditional", Blue Cross/Blue Shield PPO, most Connect Care plans and most PPOM plans. If you have another insurance program and would like to know our participation status, please call the office at (989) 631-0202.

Please give us adequate time to complete insurance/disability forms. Please be sure we have your written authorization to complete these forms.
